Employee Insurance Plans

Permanent, probationary, provisional and unclassified employees who work at least 40 hours per pay period on a regular basis are eligible to participate in the Ramsey County medical, dental, life and disability and long term care insurance plans.  (Temporary, seasonal and intermittent employees are not eligible).   For more detailed information on eligibility, enrollment restrictions, premiums, and plan detail, review the 2015 Employee Benefit Plan Reference Guide.

Medical Insurance                                                                                                                          Employee medical insurance is the HealthPartners Distinctions Plan, which provides both in-network and out-of-network coverage.  The large, open-access network is split into two benefit levels based on provider cost and quality.  The in-network office visit copay is different for employees who take the HealthPartners Health Assessment  and complete a Wellness Program and those who choose not to take the health assessment or a complete a program.  Plan benefits are summarized in the Distinctions Plan Summary Chart, in the Summary of Coverage for $20/$35 Co-Pay Plan (HA), or in the Summary of Summary of Coverage for $40/$55 Co-Pay Plan (No HA).  More detail regarding coverage is provided in the HealthPartners Group Membership Contract, the 2015 Distinctions Plan Schedule of Payments (HA), and the 2015 Distinctions Plan Schedule of Payments (Non HA).  For more information on HealthPartners services, or to search for a provider, go to the HealthPartners website, or call HealthPartners Member Services (Group#12900) at 952-883-5000.  Employees can also look at the 2015 Distinctions Tiers; however, they should always double-check the provider level with HealthPartners Member Services prior to selecting a physician. 

Dental Insurance
Dental insurance is provided by Delta Dental Plan of Minnesota.  Employees may select a point-of-service plan (Delta Premier/Preferred) or a point-of-enrollment plan (DeltaCare).  A brief comparison of coverage is offered in the Dental Plan Overview.  More detailed plan information is available in the DeltaCare Plan Summary or the Delta Premier Preferred Plan Summary.   If you have questions, call Delta Members services:  Delta Premier/Preferred (Group #138):  651-406-5916 or DeltaCare (Group #84053):  651-406-5903 or visit the Delta Dental website.

Life Insurance
Ramsey County provides basic life insurance to eligible employees in an amount equal to your annual salary up to a maximum of $50,000.  There are also several optional life insurance programs available to employees.  For more information on plans and coverage, visit the Ramsey County life insurance web page.  If you want to enroll in optional life insurance coverage, complete the Group Life Evidence of Insurability form.  You should complete and fax the form to Minnesota Life.  Ramsey County will be notified if you are approved for new or additional coverage.  Other forms you may need include the Domestic Partner Insurance Affidavit Form and Affidavit of Termination of Domestic Partner.

Minnesota Life offers several features to Ramsey county employees at no additional cost, including will preparation, (Username: lfg, Password:  resources) travel assistance, free beneficiary financial counseling, and Legacy Planning which provides assistance to employees and their families as they deal with end of life issues.     

Disability Insurance
Ramsey County provides basic long term disability insurance of 40% of gross pre-disability income for employees who become disabled for more than six months.  Employees may add an additional 20% income replacement at their own cost.   Short Term Disability Insurance is designed to temporarily replace income in cases of disability of less than 6 months at a 60% pre-disability income level.  Employees may choose a 30, 60 or 90 day elimination period.

Long Term Care Insurance
Long term care insurance provides coverage if needed for care in a nursing home or assisted living facility, home health care, adult day care and others.   This is optional coverage paid by the employee.  For more information, review the Plan Summary (pdf), visit the website for CNA (Password = munipool) or call (1-888-825-0686)  Contact your Department Benefits Representative for an enrollment packet.
