Common Mistakes on Annual Reports and How To Avoid Them
Review these areas with your report in front of you to ensure you are completing it accurately. PLEASE NOTE: If we receive your incomplete annual report, it will be returned to you and the January 31, 2014 due date still applies. If the resubmitted, completed form is not postmarked by January 31, the 10% late report penalty will be assessed.
1. Not providing numerical values for waste generated.
Provide a number in Column C – Waste Generated in 2013. It is NOT sufficient to write “same as last year”, “no change”, “yes”, etc. The number you provide in this column must indicate the actual amount of waste that was generated in 2013, which may differ from the volume you shipped.
2. Not circling the “Gal” or “Lbs” designation.
Designate the unit of measure for your waste by circling either Gallons (Gal) or Pounds (Lbs). Some common conversions are included on our instruction sheet.
3. Not providing any information in Column D - Transporter Name and EPA ID or Column E – Facility Name/Address and EPA ID.
Provide the name and EPA ID # of any hazardous waste transporters and the name, address, and EPA ID # of any hazardous waste facilities that are involved in your waste management. Some wastes are exempt from this requirement. Exempt wastes are generally shipped using a bill of lading or waste-tracking invoice.
4. Providing EPA IDs that do not match the Transporter and/or Facility.
Ensure that the EPA ID #s you provide are correct and are the ID #s for the transporters or facilities that you have listed. This information can be found on your hazardous waste manifest or by contacting the companies that you use.
5. Not providing any information in Column F – Management or Disposal Method.
Indicate what management or disposal method was used for each waste. A list of possible methods is included with the instruction sheet. This information can be found by contacting the companies that you use.
6. Not providing numerical values in Column G - Waste On-Site.
Provide the actual amount of waste remaining onsite at the end of 2013 in this column. It is NOT sufficient to write “same as last year”, “no change”, “yes”, etc.
7. Not signing the annual report form.
The annual report form must be submitted with an original signature on it. If the signature is missing, the form will be returned to you for signing.
8. Not returning the completed form by the due date.
The Annual Report must be completed and postmarked no later than January 31, 2014. If it’s postmarked after this deadline, you will be subject to a late report penalty that is equal to 10% of your total license fee.
